Passport Services Requirements

Please read the following carefully and fulfill all the requirements. Otherwise, the Embassy will not be able to process or issue new passports, renewal passports, or visas.

A. Application requirements for new passport

1. Fill New Passport Application Form (Click here to download)
2. Two recent color passport-size photographs
3. Copy of Green card (both sides) or I-94 INS papers.
4. $50.00 must be in paid by money order payable to the Embassy of Ethiopia.
5. A pre-paid envelope for return mail.
B. Application requirements for lost passport
1. Fill New Passport Application Form (Click here to download)
2. You must report the loss to the police station, obtain a letter from the station and send it to the Embassy.
3. Two passport-size photographs
4. $60.00 must be paid by money order payable to the Embassy of Ethiopia
5. Any of the following identification: copy of Green card, I-94 or INS papers.
6. A pre-paid envelope for the return mail.
C. Application for passport renewal.
1. Fill Passport renewal Application Form
2. Send in your Passport
3. Green Card copy (both sides) or INS papers
4. Renewal fee $50.00 in money order
5. A pre-paid envelope for the return mail.
